【摘要】
本計畫的目標有三：(一)瞭解臺中市老人對於社區照顧服務之需求。(二)因應人口老化趨勢，本市未來5年(2021年)老人社區照顧服務應發展之服務模式及供給量。(三)針對研究發現提出具體建議，以供本局建構完善之老人社區照顧服務體系。主要方法為：(一)文獻探討；(二)專家座談；(三)深度訪談。工作項目則包括：(一)分析臺中市老人對於社區照顧服務之使用現況及需求，蒐集服務提供者及服務使用者之建議。(二)提出臺中市未來3年、6年、9年之中長程老人社區照顧服務體系應發展之服務模式、供給量及所需人力資源等。
本研究以可以結合「生活照顧」與「健康照顧」的「整合照顧」模式，作為規劃「老人社區照顧」的概念架構，於第三章提出「整合照顧」的理論模型，第四章以次級資料和行政資料推估未來五年臺中市老人各項「生活照顧」面的需求與供給，第五章摘要專家會議和深入訪談對於現有制度的改進措施，第六章以長照2.0中廣義的「社區照顧」定義，規劃總體「社區老人整合照顧」的全貌，並依據臺中市政府個跨局處的可能分工進行建議，第七章結論與建議則偏重政治面與行政面對臺中市政府提出本研究觀察所得的具體建議。
針對臺中市政府與長照2.0策略的接軌，建議由「服務輸送整合」的核心概念切入確實執行「整合照顧」的所有項目，並且盡量透過補助民間新創產業的方式，才能收事半功倍之效，並成為未來市政府施政的亮點。本報告於最後針對市政府決策高層提出幾點關鍵性的決策建議：(一)儘速改善現有社區照顧服務單位名稱與功能多元所造成民眾難以親近的問題；(二)修改長照政策論述主軸與名稱，由「長期照顧」進展為「老人照顧」再延伸為「老人整合照顧」，以符合老人真正的需求；(三)政府在執行長照2.0政策的過程應該進一步與「整合照顧」理念結合，力求施政成果超越中央政策現有的框架格局；(四)處理「整合照顧」中「行政」層級的跨部門整合或「政府治理」問題，落實「整合照顧」理念；(五)儘速引進社區和居家企業的組織創新模式。
【Abstract】
This study aims to construct a sophisticated community elderly integrated care system in Taichung City. For reaching the goal, it is required to explore the demand and supply for community elderly care both in a current and future terms, to design an expired blueprint of integrated care, and to give an explicit policy recommendation for the officers in Taichung City Government, including some special suggestions of specific political considerations for the Mayer of Taichung CIty. The major methods of this study are literature review, focus-group meetings, and in-depth interviews of experts.
The theoretical base of integrated care is explored in Chater 3 and its empirical system construction in Chapter 6. The estimation and forecasting of the demand for and supply of community elderly care are revealed in Chapter 4. Some explicit recommendations for the current system are also presented in Chater 5. The conclusion and suggestion of this study are revealed in Chapter 7.
The most significant recommendation of this study is to construct the community elderly care system from the viewpoint of integrated care that combines health care and social care together, especially the intersetoral cooperation in the continuum of administrative level, and the integration of care delivery in the same continuum. To do this by encouraging business model brought from private sector would be more efficient than from public sector along. It is also suggested that a major political rhetoric from long-term care to elderly care, and further, to elder integrated care is necessary for clarifying its ultimate value of community elderly care. |
